Welcome to Moms & Minis Wellness Co. These Terms & Conditions apply to the use of this website and to services purchased or scheduled through Moms & Minis Wellness Co. By using this website, scheduling services, purchasing services, or enrolling in a membership, you agree to these Terms & Conditions.

Healthcare Services

Moms & Minis Wellness Co. provides pediatric and wellness services, including in-home visits, telehealth services, memberships, parent coaching, and other services offered by the practice.

Availability of any particular service, treatment, prescription, test, referral, or procedure is subject to clinical assessment, medical necessity, the provider’s professional judgment, applicable laws and regulations, and the scope of services offered by Moms & Minis Wellness Co. Memberships

Membership in Moms & Minis Wellness Co. is voluntary and is not required to receive care. Patients may receive services on a self-pay, non-member basis at the applicable rates. Membership_Agreement_Questionnaire.pdf

Membership benefits and pricing vary by plan. Services not specifically included in a selected membership may result in additional charges. Membership fees and visit fees are separate unless a particular visit is expressly included in the selected membership plan. Membership_Agreement_Questionnaire.pdf

Patients enrolling in a membership are also subject to the separate Membership Agreement provided by Moms & Minis Wellness Co.

Membership Billing

Membership fees are automatically billed to the payment method maintained on file. By enrolling in a membership, the patient or responsible party authorizes recurring charges according to the billing schedule established at enrollment. Membership_Agreement_Questionnaire.pdf

Patients are responsible for maintaining a valid payment method.

Pay-Per-Visit Services

Patients who are not members may receive eligible services at the applicable non-member rate. Members may also incur separate charges for services that are not included in their membership.

Additional services, testing, laboratory fees, imaging, medications, third-party services, and other services not specifically included in a membership or care package may result in additional charges. Membership_Agreement_Questionnaire.pdf

Cancellation of Membership

A patient or legal representative may request cancellation of a membership at any time.

Cancellation requests must be submitted in writing and require at least 30 days’ advance written notice. Membership fees continue during the applicable notice period. Membership_Agreement_Questionnaire.pdf

Refund Policy

Membership fees are generally non-refundable once the applicable membership period has begun, except when a refund is required by law or otherwise provided under the Membership Agreement.

Unused membership benefits, visits, texting availability, discounts, educational materials, or other membership benefits have no cash value and do not roll over from month to month unless expressly stated otherwise. Membership_Agreement_Questionnaire.pdf

Appointment Cancellations & No-Shows

Appointments canceled with less than 24 hours’ notice may be subject to the applicable visit fee. A no-show may also be charged the applicable visit fee.

These charges are separate from membership fees, and membership benefits may not be used to satisfy cancellation or no-show charges.

Communication

Membership plans that include texting provide communication through Spruce during designated business hours.

Membership does not guarantee immediate responses or 24-hour medical availability. Spruce and other electronic communication methods should not be used for medical emergencies. Call 911 or seek emergency medical care when appropriate.

Business Hours, Holidays & Practice Closures

Moms & Minis Wellness Co. is a solo-provider practice. Services, appointments, and member communication are available during established business hours and are subject to provider availability. The practice may periodically close for holidays, vacations, continuing education, illness, family obligations, or other scheduled or unexpected circumstances. For planned extended closures, Moms & Minis Wellness Co. will make reasonable efforts to provide patients and members with at least 7 days’ advance notice. When possible, additional notice will be provided for holidays and vacations known well in advance. During a practice closure, routine messages, appointment requests, prescription refill requests, forms, laboratory result questions, and other non-urgent requests may not be reviewed or addressed until the practice reopens. Patients are encouraged to plan ahead for routine needs when advance notice of a closure has been provided.

Membership fees are not prorated or refunded for holidays, weekends, scheduled practice closures, or periods when the provider is unavailable, as membership fees support ongoing access to the practice and the overall services and benefits of membership.

Moms & Minis Wellness Co. does not provide 24-hour or emergency medical coverage. Patients experiencing an urgent medical concern during a practice closure should contact their primary pediatrician or an appropriate urgent care provider. For a medical emergency, call 911 or seek care at the nearest emergency department.

Insurance

Membership is not health insurance.

Moms & Minis Wellness Co. does not bill health insurance for healthcare services covered by the membership. Patients may request an itemized receipt for eligible separately paid services and may independently submit it to their insurance company for possible reimbursement. Reimbursement is not guaranteed. HSA/FSA

Patients may use an HSA or other healthcare spending account for eligible healthcare expenses when permitted by their plan and applicable law.
